Borrowing Policies

LIBRARY CARDS

Anyone who lives, works, attends school, or owns property in Ohio may apply for a library card. Your library card may be used at any of the following:

  • Middletown Public Library
  • Trenton Library
  • West Chester Library

To borrow materials, you must have a Middletown Public Library card. You will need to present your card each time you wish to check out materials. If you forget your card, you may purchase a temporary card for 25 cents, good for one visit only. Identification will be required.

The charge for replacing a library card is $2.00.

APPLICATION PROCEDURE

Fill out the application form completely.

Children are eligible for a card at birth. Children under 14 must have a parent or guardian with them when applying. When a child is not present, a parent or guardian must present identification for the child to apply for the child's card. Each applicant (parent or guardian of child applicant) must show a photo I.D. with a current address or 2 pieces of identification, at least one of which bears the current address of the applicant.

STATEMENT OF RESPONSIBILITY

By acceptance of your library card you agree to the terms of issue and assume responsibility for the card's use prior to its surrender to the Middletown Public Library. The holder accepts liability for the unauthorized use of the library card up to the amount of $100.00. You will not be liable for unauthorized use which occurs after you give notice of its loss. Immediately notify the Library in the event of loss, theft, or possible unauthorized use of the card. The card is the property of Middletown Public Library and you agree to surrender it upon request. If the card is issued to a person under 18, the parent or guardian is responsible for the selection, use, return of library materials and for any charges incurred.

BORROWING MATERIALS

 

In order to make the public library useful to the entire community we must all share the resources in a responsible manner.

 

Customers may have on loan at any one time a total of the following audio visual formats:

  • 2 Video Games
  • 12 CD-ROMs
  • 12 Read-Alongs
  • 12 Audio Books (on cassette or CD)
  • 12 CDs
  • 12 Video items in either VHS or DVD format. Video materials may only be borrowed on an adult library card, which is available for customers 18 years old or older.

The library cannot assume responsibility for damage to video, audio, or computer equipment through the use of library materials.

LOAN PERIODS

  • 21 DAYS - Books, Magazines, Audio Books, CDs, Read-Alongs and CD-ROMs
  • 7 DAYS – Videos, DVDs and Video Games

RESERVING MATERIALS (Placing a Hold)
If the item you want is not on the shelf, you may place a hold at the on-line catalog or via the library’s website. You may also request assistance at the Information Desk.

 

You will be notified when the item is available. Videos and DVDs may not be reserved.


RENEWING MATERIALS
You may renew materials via the library’s website, by phone, or at the library. Videos and DVDs may be renewed once. Video Games may be renewed once if they are not on reserve. All other materials may be renewed twice if they are not on reserve. For more information, please refer to the Library’s Renewal Services.


REFERENCE MATERIALS
Reference materials do not circulate. These items are kept in the library building for the use of all customers.

INTERLIBRARY LOAN
Items not owned by the library system can often be borrowed from another library through Interlibrary Loan. There is a base charge for this service. Please refer to the Library's Interlibrary Loan Services for additional information.

Lost or Damaged Materials

Damaged plastic book cover   $2.00
Video/DVD/Video Game case   $2.00-$7.00
Media bag   $6.00
CD case   $1.00
Audio Book case   3.00 - $25.00
CD-ROM case   $8.00

The same rates apply to adult, young adult and juvenile materials. Visa and MasterCard accepted.

If materials are 180 days overdue, the library will no longer accept the return of the items. You will be charged the original cost of these materials.

Money will be refunded for lost materials returned within 180 days of the due date. Overdue costs will be deducted from the amount refunded.

The library cannot accept replacement items from customers for lost or damaged materials.

If materials are overdue, return them as soon as possible to avoid additional overdue fines. When charges exceed $10.00, you may not renew or check out additional materials. Uncollected accounts in excess of $50.00 will be turned over to Unique Management Services. A processing fee of $10.00 will be added to accounts turned over to Unique. Customers with unpaid fines who have been turned over to Unique Management Services shall not be allowed to borrow materials and are not permitted to use library services which require a library card, such as the Internet, until the balance is paid in full.

There is a charge of $30.00 on checks returned for insufficient funds.
